In the 2-12-13 issue of The Lincoln County Journal, the Winfield Kindergarten registration was listed with incorrect information. Below is the corrected information for the registration times.
Winfield R-IV School District will be holding its Kindergarten Registration & Screening for the 2013–2014 school year. Please call 668-8195 ext. 0 for an appointment. It will take approximately an hour for your child to complete the screening process. The dates are as follows:
Tuesday, April 16, 2013 (evening)
Thursday, April 18, 2013 (evening)
Saturday, April 20, 2013 (all day)
If you have a child turning 5 years old on or before July 31, 2013 or if you know someone who has a child turning 5 years old, they need to be screened and registered. No child will be screened or registered if they are not 5 years old on or before July 31, 2013. The following information is needed to register a child for kindergarten:
1. Proof of Residency (sales contract, lease agreement, or utility bill)
2. State Issued Birth Certificate (can be obtained from the Lincoln County Health Department)
3. Immunization record – Shots needed: 4 DTP (last one must be after 4th birthday), 3 Polio (last one must be after 4th birthday), 2 MMR, 3 Hep B & 2 Varicella (chickenpox) or proof of disease (must be a written letter from child’s physician)
4. Emergency Contact Numbers to be placed on registration forms
5. Social Security Number
